In addition to the four places at which one is required to bow in the Amida, there is a FIFTH place at which one bows-- עושה שלום במרומיו at the end of the Amida. HaRambam states (Hilkhot Tefillah 5:10): "...Upon completing the Amidah, one bows and takes three steps backwards while bowing. He bids farewell [i.e. bows his head] from his left and afterwards from his right. Then, he lifts his head up from the bowed position."
Additionally, there is a custom brought down by the Magen Abraham (Orah Hayyim 132:2) to bow in עלינו לשבח at the words ואנחנו משתחוים. The custom is to bow SLIGHTLY (not a complete bow like in the Amida), just enough so that one doesn't say the words "ואנחנו משתחוים, and we bow..." without bowing. This is different than the other places where Maran mentioned not to bow since those places do not mention the words "and we bow" like in עלינו לשבח.
